The badge consists of a shiny brass bar pin, ladder bar, and pendant. The bar pin has a rectangular base with a crest formed by flags and clouds on top of the base. "MEMBER" is in raised letters on the base. A Third National Confederate pattern flag is on the left side of the crest, enameled in red, white, and blue. An Army of Northern Virginia pattern flag is on the right side of the crest, enameled in red, white, and blue. An oval relief bust portrait of Robert E. Lee is in the middle of the clouds. "GENERAL R. E. LEE" is in raised letters above the portrait at the top of the oval. A clasp pin is soldered to the back of the bar. Small metal rings link the bottom of the bar pin to the top of the ladder at each end. The brass ladder bar is shaped like a banner with scrolled ends and has an enameled base. "FORREST / CAVALRY / CORPS" is in raised black letters on the banner. Small metal chains connect the bottom of the ladder bar to the top of the pendant on each side. The pendant is an ornate brass frame around a circular pewter insert. The obverse of the pewter insert contains a relief bust portrait of Nathan Bedford Forrest in the center. "GENERAL N. B. FORREST" is in raised letters above the portrait along the top edge of the insert. The reverse of the insert contains a relief image of a Confederate Third National pattern flag in the center. The badge is attached to the original white paper display card from the manufacturer. The manufacturer's information is printed in black at the bottom of the card below the badge and reads, "Made by The Schwaab Stamp & Seal Co., Milwaukee."
