Form is mostly pre-printed with blanks filled in with handwritten information. Oath reads: "Commissioner's Office / Nashville, Tenn, February 24th 1864 / I do hereby Certify, That on the 24th day of February 1864 at Nashville Davidson County Ten, the oath prescribed by the President of the United States, in his Proclamation of December 8, 1863, was duly taken, subscribed, and made matter of record by D. Weaver of Davidson County. The same being No. 496 of the book of said County. / E.R. Campbell [signature] / Commissioner."
A five cent George Washington stamp is affixed to the lower left corner.
